This session will examine the efforts of the National Interoperability Collaborative (NIC), Michigan Health Information Network (MIHiN), Office of the National Coordinator (ONC) and the MITA Technical Architecture Committee (TAC) in utilizing standards such as Fast Healthcare Interoperability Resources (FHIR) and the National Information Exchange Model (NIEM) to form the foundation of a person-centered suite of solutions.  We will discuss our vision and our efforts to make interoperability a reality and how this vision can impact people.  The speakers are participating in a Proof of Concept (PoC) initiative called Project Unify that is open to all.  Project Unify is intended to demonstrate, educate and push the art of the possible in serving people in H&HS.
